Prospective Students: Application

Thank you for your interest in our program. Applications for Autumn 2009 are now closed. Applications for Autumn 2010 may be submitted online at the Graduate School Application website: https://www.grad.washington.edu/applForAdmiss.

The deadline for completed applications is January 5, 2010.

Important Note for International Applicants: The requirements for English language proficiency have changed. A Master's degree from a U.S. (or other English-language) university will no longer exempt an applicant from submitting a TOEFL (or IELTS) score. Please see below for more information.

SOPHAS: Applicants may also use the Schools of Public Health Application Service (SOPHAS) to apply. Please note that we do not require applicants to use SOPHAS for our program. SOPHAS applicants will need to submit an application and fee via the Graduate School Application website in addition to the SOPHAS application and fee. SOPHAS applicants will also need to submit official GRE scores to our Graduate School, and international applicants will need to send official transcripts to both SOPHAS and our Graduate School. The deadline is the same for SOPHAS applicants: January 5, 2010.
 
 
A COMPLETE APPLICATION consists of:

  • Online application and payment of $65.00 application fee
  • Submission of Statement of Purpose
  • Submission of GRE score (all applicants; must be taken within five years of application), and TOEFL or IELTS score (most international applicants*)
  • Submission of three letters of recommendation**
  • Optional: submission of resume if seeking a financial assistantship (RA/TA, Traineeship)
  • Hard-copy submission of OFFICIAL transcripts to the department (international applicants must also send hard-copy transcripts to the Graduate Admissions Office)
  • Applicants using SOPHAS should follow the instructions on the SOPHAS website.

    * The following are exempt from taking the TOEFL or IELTS: Applicants who hold a bachelor’s degree from a regionally accredited institution in the United States or from an institution in Australia, the Bahamas, Canada, Ireland, New Zealand, Singapore, Jamaica, Trinidad and Tobago, or the United Kingdom.
    OR applicants who can provide documentation verifying that their undergraduate degree is from an institution (in a country other than those listed above) where all instruction is in English. For all others, a TOEFL (Test of English as a Foreign Language), IELTS (International English Language Testing System), or MLT (Michigan Language Test) is required. Test scores are valid for two years and must be valid on the date the application is submitted. For more information on English language requirements, please see the Graduate School's Memorandum No. 8.

    ** If your recommenders prefer to send hard-copy letters, they should be addressed to the Department at: Graduate Program in Biostatistics, Campus Box 357232, University of Washington, Seattle WA 98195-7232. We STRONGLY encourage recommenders to use the online submission of letters through the Graduate Admissions application website.
